You are requested to send us your regular mailing address and fax number in order for us to help you obtain your needed visa. We will fax you an application form for the Visa Entry request where you will need to fill in the necessary information. Upon completion, please fax (972-4-9081218) the form to us by March 20, 2003 along with a copy of the page from your passport, which includes your photograph and personal details. Your visa will be sent to the Israeli Consul closest to your residence about a month prior to the conference. If you will not be at your permanent address kindly inform us of your mailing address in order to send you the visa.
